New Online System to help Pregnant Women self refer to Midwife

Congratulations if you have recently found out you are pregnant!

Did you know you can now use the self-referral online system to contact your midwife?

Use the link below to access the form

https://www.patientportal.lincolnshire.nhs.uk/

Following receipt of your referral you will be contacted within a week by a member of the community team and given an appointment. To help provide you with the best possible care, we would like to get you booked in to see a midwife before you are 8-10 weeks pregnant.
