Run and managed by the NHS Lincolnshire Adult and Older Adult Community Mental Health Transformation programme, this includes hobby groups and support networks, access to practical support such as housing and debt advice as well as Night Light Cafes and Community Hubs.

The aim of the website is to:

  • Make it easier for people and/or their carers to access to mental health and wellbeing support services in a community setting.
  • Support people to self-manage their own health, where appropriate.
  • Help people with long-term conditions and their carers benefit from access to supplementary non-clinical support options.
  • Help people become less socially isolated and more independent which improves mental health and wellbeing.

If you know anyone who is presenting with mental health challenges, who will benefit from support within the community, please let them know about H.A.Y.
